Final payment for surgery will be required at the pre-operative appointment, as per our surgery terms of agreement.

Regarding Prosper Healthcare Funding Loans

Funds are transferred directly into your designated bank account. Payment for surgery is not automatically provided to our office. It is the patient’s responsibility to provide the funds during the pre-operative appointment.

Regarding CareCredit and Alphaeon Payments

The actual cardholder is required to be physically present at the time the CareCredit / Alphaeon is charged. Our office is required to verify the cardholder and obtain two forms of ID. An “authorized user,” as declared by CareCredit or Alphaeon, may also complete the transaction with two forms of ID.
