VISITOR POLICY!
Only patients with a scheduled appointment may enter the facility.
Those accompanying patients may not enter. We will call in, if necessary. (Parents of minors are an exception.)
A face mask or covering is required to enter the office.
Maintain social distancing by sitting/standing 6 feet apart. If the waiting room is full, patients will be asked to wait in their vehicle or outside until called in.
Do not enter the facility if you have or have had any of the following symptoms within the last 2 weeks:
- Cough
- Fever
- Diarrhea
- Severe fatigue
- Conjunctivitis
- Discoloration of toes or fingers
- Loss of sense of taste or smell
- Muscle aches or pains
- Nasal congestion/runny nose
- Shortness of breath
- Tightness in the chest
Do not enter if you have been in contact with anyone who has had these symptoms or thinks they may have had COVID-19 within the past 2 weeks.
IF YOU HAVE ANSWERED “YES” TO ANY OF THE ABOVE SYMPTOMS, DO NOT ENTER THE FACILITY.
PLEASE CALL OUR OFFICE at (818) 500-0823 FOR APPROPRIATE DIRECTION.
In accordance with all CDC guidelines, our office will be taking the following extra precautions:
- Frequent hand-washing and use of alcohol hand sanitizer.
- Wearing appropriate attire and protective equipment.
